Office Policies:


 

At Elle MD we spend a significant amount of time with each patient.  This allows us to provide the attention, quality, comfort, and compassionate care that our patients need. In order to achieve this level of care, the following office policies are in place with no exception.


  • No Show/Cancellation Fee
  • A $50 fee will apply for no shows, cancellations or reschedules with less than 24 hours notice. Please call or use the online scheduling page to cancel and reschedule your appointment. 
  • More than 3 no shows or late cancellations (any appointment types) will result in termination from the practice.
  • No show fee is due from previous cancellations before the next appointment can be scheduled.
  • Cancellation - You may cancel/reschedule your appointment without penalty up to 24 hours before your appointment. Voicemail messages or messages via our text messaging system are acceptable.

 

New Patient Information


 

  1. Please complete all required forms in advance of your visit. We will send all forms electronically to your email through Jane app once your appointment is scheduled. The documents and questionnaires are very detailed and will take about 20 minutes to complete. Please complete your new patient forms 2-7 days prior to your visit, as this will allow us time to review your information.
  2. Please answer all of the questions thoroughly and accurately, as this allows us to provide you with the best quality service during your visit. If your questionnaire is not complete, your appointment will be rescheduled. 
  3. If you had blood tests drawn in the last six months, bring a copy to your first visit. We will review your labs and if needed, will order additional testing.
  4. For patients in the Weight Management Program, we request that you have an EKG done within the last 90 days. If you have not had an EKG done in the previous 90 days, we may recommend one during your visit. You will be charged for the EKG on the day that the EKG is done.
  5. Please wear a short sleeve shirt to your visit. We will be checking your blood pressure.
  6. Please arrive on time for your appointment.

  • Do you accept insurance?

    We do not participate in any commercial insurance plan, PPO or HMO. We also do not participate in any Medicare Advantage or Medicare Part C plan. If you have Traditional Medicare coverage, this means that for only Medicare covered services, your financial responsibility and payment for those services generally will be limited to the standard 20% coinsurance or copayment. 

  • What payment methods do you accept?

    We accept payment by cash, check, VISA, Mastercard, American Express and Discover.  We also accept most FSA/HSA

  • Can I use my flexible spending or health savings account?

    Yes. We accept most HSA and FSA cards.

  • What if I need to reschedule my appointment?

    We understand that your schedule might change, and you might need to reschedule your appointment.
